Just login to citicards.com, send them a message asking for photo card form (specify how man you want).

Two note:

1) after you get the photo card, your existing card is still OK. so you can use either one.

2) when sign the form, sign with a wider pen. Because now, your signagure will appear in the front of your card. Use a thin pen will make it less visible.
